Course Objectives

This course helps those who work in health, social care or children and young people’s settings to understand how to implement personalised care planning and learn about the process, the journey, and the skills needed to make it a success.

Learning outcomes

At the end of this course, learners must be able to:

  • Gain good confidence to plan care for individuals

  • Understand how to update care planning

  • Understand how to incorporate individual’s needs in care planning

  • What do we record in care plan and why?

  • How do we record in care plan and what to consider

  • What information do we need to do successful care planning

  • Who are the contributors to care planning?

  • Person-centred care planning

This course can be assessed by a wide range of methods, depending on the level and mode of study. Methods available include but not limited to the following:

  • Written Assignment

  • Observation

  • Multiple choice question (online only)

  • Professional discussion

  • Witness Testimony
